Hosted the Hybrid Simulation Modelling Lab public forum on healthcare in Clare, chaired by Clare Champion journalist Dan Danaher, with eleven of the twenty General Election candidates in attendance , Eddie Punch, Kevin Hassett, Cathal Crowe TD, Leonora Carey, Senator Timmy Dooley, Cllr Rita McInerney, Amanda Major, Dr Tom Nolan, Hilary Tonge, Violet-Anne Wynne TD, and Michael Leahy contributing to a constructive discussion on regional health challenges and solutions.

Locally referred to as the Model 4 hospital Study, this groundbreaking research is significant as it is the first to recommend the construction of a new 352-bed Model 4 hospital in Clare on a greenfield site, moving beyond legacy calls to reinstate the former Emergency Department at Ennis General Hospital. The study presents a strategic vision to address urgent capacity needs and strengthen acute care provision across the Mid-West, offering a transformative, future-focused solution designed to deliver comprehensive high-level acute services, ensure long-term resilience, and improve health outcomes for the community.

Model 4 Hospital study


A new study has recommended the construction of a new 352-bed Model Four Hospital with an Emergency Department on a green field site in Ennis. The study is entitled ‘Preliminary Quantitative Analysis and Strategic Approach to Emergency Department Overcrowding at University Hospital Limerick, Ireland’s Health Service Executive, MidWest Region’. It has been completed by Ennis and University of Limerick PHD student, Sinéad Madden. According to the study, the building of such a Model Four Hospital in Ennis is necessary to deal with the county’s long-term growing healthcare needs.

The presentation outlining the need for a new model four hospital by 2031 in Ennis, was presented at the prestigious International System Dynamics Conference in Boston this week.
It comes on a day when nurses and midwives have recorded 73 patients on trolleys at University Hospital Limerick.
Independent Academic Sinéad Madden , who presented the findings says the need for commitment to a model four hospital in this county is well overdue.

An International Conference has heard the case for a new hospital in Clare.
The facility would be a Geriatric Centre of Excellence, equipped with an emergency department and would be located on a greenfield site, while a second Model 2 hospital and Local Injury Unit is suggested for West Clare.
The recommendations are based on studies of the MidWest which analysed where future investment would have the greatest impact and were conducted by Hybrid Simulation Modelling Labs.
Founder Sinéad Madden says it’s needed to meet future population growth.

A proposal intended for inclusion in the next National Development Plan has been made for a ‘University Hospital Ennis’




Met with Bernard Gloster, CEO of the HSE, during the 2024 HMI Conference, where the Madden Model 4 Study , a quantitative preliminary analysis addressing acute hospital capacity in the Mid-West was presented. In an era defined by volatility, uncertainty, complexity, and ambiguity (VUCA), the conference brought together leaders, innovators, and practitioners to explore adaptive leadership, digital transformation, and collaborative strategies for delivering high-quality care in a rapidly evolving healthcare landscape.

Featured in national media for leading the Madden Model 4 Study, the first quantitative analysis to recommend a new 350-bed Model 4 hospital in Ennis to address critical capacity shortages and reduce overcrowding at University Hospital Limerick. Conducted in response to a request from the Minister for Health, the study found that reallocating acute care capacity to Clare would bring Limerick’s hospital services in line with its population needs. Based on conservative national bed-to-population ratios, the research highlighted significant service gaps, long travel distances for rural patients, and the need for complementary facilities in West Clare. The findings have sparked national debate ahead of the forthcoming HIQA report and have been recognised as a pivotal contribution to healthcare planning in the Mid-West.


Attended the Health Summit 2025 at Croke Park, Dublin (6 February), a national forum focused on transforming how healthcare is planned, delivered, and experienced. The event brought together policymakers, healthcare leaders, clinicians, and innovators to share strategies for system reform, patient-centred care, and the integration of emerging technologies into health service delivery. Met with Robert Watt, Secretary General of the Department of Health, and Bernard Gloster, CEO of the HSE, during national health leadership events, discussing strategic planning, capacity challenges, and policy priorities.

Attended the Future Health Summit at the Royal Dublin Convention Centre (28–29 May 2025), featuring Minister for Health Jennifer Carroll MacNeill TD. The event brought together over 600 senior healthcare leaders and international experts to discuss challenges and opportunities in Irish healthcare, showcase innovations, and foster industry collaboration. Presented the MIDCARE.
Met with former Minister for Health, Stephen Donnelly, during the Future Health Summit 2025, where he was in attendance alongside national and international healthcare leaders.
Participated in the SPHeRE Network 11th Annual Conference (4 March 2025, RCSI), themed “Translating research to health policy and system change.” The event highlighted how high-quality academic research can inform and shape health policy, system reform, and population health outcomes. Featuring leading voices such as Prof Lucy Gilson, Róisín Shortall, Prof Vera Morgan, Prof Rose Galvin, and Prof Ian Underwood, the conference explored interdisciplinary collaboration, research leadership, and strategies for translating evidence into practical, impactful health system change.

At the International System Dynamics Conference 2022 at the Frankfurt School of Finance & Management, I had the privilege of meeting three giants of the field: George Richardson, John Sterman, and Jørgen Randers. It was inspiring to see Richardson reflect on Jay Forrester’s legacy, Sterman extend the field into pressing global and business challenges, and Randers bring his long-term perspective on sustainability and the future of our world. Meeting them all in one place made Frankfurt a truly unforgettable gathering.
